Little Legends Occupational Therapy is a private occupational therapy practice under the Directorship of Luke Casey.

What are your Legal Obligations?

In order to provide you with the health care services that you have requested, we will need to collect and use your personal information. If you provide incomplete or inaccurate information to us or withhold personal and health information from us, we may not be able to provide you with the services you have requested.

What information does Little Legends Occupational Therapy collect?

We only collect information from you that is necessary to provide you with occupational therapy services and manage our relationship with you. The information we collect includes; your name, date of birth, address, health fund details and information about your health history and family history.  We require this information in order to provide you with the best quality service possible.

How does Little Legends Occupational Therapy collect my health information?

We will usually collect your health information directly from you. Sometimes, we may need to collect information about you from a third party such as a medical practitioner, another health service provider or educational setting.

How does Little Legends Occupational Therapy use your information?

We will use your personal information for the purpose you have given the information to us. We will use your information to provide occupational therapy services to you, to manage our relationship with you and to contact you in relation to matters concerning your care. We may also use your information for other purposes permitted under the Privacy Act 1988.

Who might Little Legends Occupational Therapy disclose your information to?

We may disclose your information to the following people:

(a) Disclosure to other health professionals involved in your treatment

Your personal information will generally only be used by the therapist involved in your care, however on occasion your care may be provided by a number of health professionals or consultants (for example your medical practitioner, speech pathologist, and/or psychologist) working together. We may disclose your information to these health professionals and consultants as part of the process of providing your care.

(b) Other uses and disclosures

In order to provide the best possible environment in which to treat you, we may also use or disclose your personal and health information where necessary for:

Activities such as quality assurance processes, accreditation, audits, risk and claim management, patient satisfaction surveys and staff education and training, invoicing, billing and account management.

To liaise with your health fund, Medicare or the NDIS and where required, provide information to your health fund, Medicare or the NDIS to verify treatment provided to you.

The purpose of sending you standard reminders, for example for appointments and follow-up care, by text message or email to the number or address which you have provided to us.

(e) Other use with your consent

With your consent, we may also use your personal information for other purposes such as providing you with additional information about our services or research. Please know, however, that unless you provide us with your express consent for this purpose, we will not use your information in this way. We will not disclose your personal information to any individual outside Australia.

Can you have access and correction of your health information?

You have the right to access the personal and health information that we hold about you. You can also request an amendment to your personal and health information should you believe that it is inaccurate.

If we do not agree to change your medical record/personal information in accordance with your request, we will permit you to make a statement of the requested changes and we will enclose this with your record.

Should you wish to obtain access to or request changes to your health record, please contact Little Legends Occupational Therapy and ask for our Privacy Officer who can give you more detailed information about our access and correction procedure.

Please note that Little Legends Occupational Therapy may recover reasonable costs associated with supplying this information to you.

Data Quality & Security

Little Legends Occupational Therapy will take reasonable steps to ensure that the personal information we collect, use or disclose is accurate, complete and up-to-date.

We will take reasonable steps to protect your personal information from misuse, interference, loss, unauthorised access, modification or disclosure. We use technologies and processes such as access control procedures, network firewalls, encryption and physical security to protect your privacy.

We will destroy or permanently de-identify any of your information which is in its possession or control, and which is no longer needed for the purpose of which it was collected, provided we is not required under an Australian law or court/tribunal or otherwise to retain information.

Fees and Funding

Purpose

At Little Legends Occupational Therapy, we are committed to providing clear and upfront information about the fee structure for our services. It is the intention of this policy to provide this information to our clients and potential clients, prior to engaging our services.

How Much will our Services Cost?

Little Legends Occupational Therapy will charge for services at the hourly rate as prescribed by the NDIS Pricing Arrangements. The current NDIS rate is $193.99 per hour.

Clients will be invoiced following the completion of services. The payment of invoices is required within 7 days .

Non Face to Face Support

At Little Legends Occupational Therapy, we take time to prepare individually for each client. In addition to this preparation, we may require additional time to prepare documentation, liaise with other health care providers and consultants. The fees charged for this preparation and support will be charged in addition to the hourly session rate and in line with the NDIS Pricing Arrangements.

Travel

Little Legends Occupational Therapy provides services in different environments to meet the different needs of our clients and their families. These services may be provided in our consulting rooms, at your home, your school, your local community or via videoconferencing.

We recognise the best practice in early intervention is to deliver services in the natural environment and therefore you may choose to receive services in a place that works best for you.

If you elect to receive a service outside our consulting rooms, your therapistwill be required to travel to deliver this service.

Charges for Travel

If a therapist travels to provide services to you, you will be charged for the travel to and from the location where the service is provided.

Travel will be charged at actual travel time at the standard NDIS rate.

The NDIS Pricing Arrangements outlines when providers can charge for travel. The service items that providers can claim travel costs for are listed in the NDIS Support Catalogue.

Private Health Funds

Occupational Therapy sessions are claimable from most private health care funds. To see if your private health care covers Occupational Therapy, please contact them regarding their rebates on the following codes 100, 200, 250 and 300.

Medicare

An Allied Health Care Plan is a plan which allows you to claim a Medicare rebate on up to 5 sessions in a calendar year.

You can ask your GP if your child is eligible for this plan. This plan gives a rebate of $56.00 for each session (as of September 2022). Please note that the gap must be paid.

NDIS

Little Legends Occupational Therapy works with clients who are participants of the National Disability Insurance Scheme (NDIS). At this time, we only provide services to NDIS participants who are self-managed or plan-managed.

Cancellations

At Little Legends Occupational Therapy, we take time to prepare individually for each client, and see only a certain number of clients each day to ensure that we are providing the best possible service for each family.

We acknowledges that our clients and their families face multiple demands on their time and that appointments may need to be cancelled on occasion. Cancelling your appointment ahead of time allows us to reschedule another client for the session time.

Cancellation fees are not intended as a penalty however they are necessary to ensure that we can operate efficiently and provide all our clients with a quality service.

Cancellation Fees

If you are unable to attend your appointment, please contact us as soon as possible and at least 48 hours before your scheduled appointment to avoid a Cancellation Fee.

In the event that you do not provide us with 48 hours’ notice, or you fail to attend the appointment, a Cancellation Fee will apply.

The Cancellation Fee will be charged at 100% of the value of the planned support, including any travel for the session.

In the case of an emergency, the Cancellation Fee may be waived at your therapist’s discretion.

Please note that our Cancellation Policy must be read and acknowledged before their first appointment will be confirmed.

In the event that Little Legends Occupational Therapy need to cancel your appointment, we will provide you with as much notice as possible. This would only occur in exceptional circumstances, such as your therapist being unwell. In this instance, your appointment will be rescheduled to the next available appointment.

Code of Conduct

All therapists are current members of The Australian Health Practitioners Regulation Agency (AHPRA) and therefore covered by the AHPRA Code of Conduct (the code).

It sets out the standards of professional conduct the National Boards expect and is used by Boards to evaluate therapists’ conduct. Occupational Therapists have a professional responsibility to be familiar with and to apply this code.

If you are receiving care or treatment from one of our therapists, they are covered by this code and it will help you understand the behaviour you can expect.
